Azure mysql user connections with proxy sql

I’ve looked for a couple of days, and can’t find a definitive answer. I have an application running on a kubernetes cluster that wants to connect to an Azure Database for MySQL server via proxysql. The Azure service requires user names in the format user@AzureDBName as described here:

My application generates usernames based on the origin host. (user@OriginHostIP)
I’ve read that proxysql can accept requests from user a, and relay them to MySQL with user b. I’ve read through the proxysql documentation, and can’t find an explicit example. Has anyone done this? Thanks.
